Start off by making a list of internal and external consequences. Here's and example from the book:
Then identify where you procrastinate the most. i.e. home (chores), school, work, personal care.
Think back to a specific time you remember procrastinating on a task.
Write down excuses that you find yourself saying.
Take a task one step at a time. Don't think of the big picture. Rather than thinking that you have to finish reading a book a month, make a small goal to read a chapter, then reward yourself for following up on that goal by going on the internet or enjoying social activities.
